A reading of 150/80 has a pulse pressure of 70 mmHg. Your monitor may show 150 and 80, but it usually leaves the subtraction to you.

That extra number can add context, particularly when the gap stays wide or narrow across a series of careful readings. Pulse pressure tends to increase as the large arteries become less flexible, so it commonly rises with age. It can also change because of posture, activity, stress, illness, cuff fit, or an irregular measurement.

Pulse pressure formula: one subtraction

Pulse pressure is the systolic number minus the diastolic number.

For a reading of 128/76:

128 - 76 = 52 mmHg

For a reading of 118/78:

118 - 78 = 40 mmHg

Use numbers from the same measurement. Do not subtract a morning systolic value from an evening diastolic value, even if both readings came from the same week. The subtraction is meaningful only when the two pressure values were recorded together.

Pulse pressure is also different from pulse rate. Pulse rate is the number of heartbeats per minute; pulse pressure is measured in millimetres of mercury, written as mmHg. A monitor can display both, but they describe different features of circulation.

What does pulse pressure indicate?

Pulse pressure describes the size of the pressure swing between the heart’s contraction and the resting phase between beats. Large arteries normally expand when blood is ejected and recoil afterward. That expansion and recoil help smooth the flow of blood through the circulation.

As arteries become less elastic, they do not expand as readily during each heartbeat. Systolic pressure may rise, while diastolic pressure stays similar or falls. The resulting gap becomes wider. This is a physiological explanation for the age-related rise in pulse pressure, not a way to identify the cause of a particular person’s reading.

The pattern is especially relevant in later adulthood because arterial stiffness becomes a more common influence on systolic and diastolic pressure. Age does not make every wide reading harmless. It changes the background against which a repeated result is interpreted.

Research across groups of people has associated persistently wider pulse pressure with higher cardiovascular risk, particularly in older populations. The direction of that evidence is consistent enough for pulse pressure to be discussed as a risk marker, but it is not precise enough to use one home reading as a personal forecast. Population associations do not reveal what will happen to an individual.

Systolic and diastolic pressure remain the principal numbers in blood-pressure classification. Pulse pressure is a supplementary calculation. A pulse pressure of 70 mmHg, for example, does not tell you whether the original reading was 150/80, 170/100, or 110/40. Those readings have different systolic and diastolic patterns even though the first two produce different gaps and the last illustrates a very different context.

Wide pulse pressure: why the gap grows

There is no single international cutoff that defines a wide pulse pressure. In routine clinical discussion, a repeated value above about 60 mmHg is often called wide. Other references use thresholds near 50 mmHg or describe the value in relation to age and the underlying blood pressure. These are reference points, not diagnoses or personal targets.

Arterial stiffening is the main age-related explanation. Long-standing high blood pressure, diabetes, kidney disease, atherosclerotic disease, and certain valve or circulation problems can also alter the relationship between systolic and diastolic pressure. Pulse pressure alone cannot distinguish among these possibilities.

A wide result can be temporary. The following can affect the underlying blood-pressure reading and therefore the subtraction:

For example, 152/88 gives a pulse pressure of 64 mmHg. That is wide by a common rule of thumb, and the systolic number also falls into a high category under both major office frameworks. One reading does not establish hypertension. Repeated readings taken under comparable conditions provide a more reliable pattern.

A reading of 138/70 gives a pulse pressure of 68 mmHg. The gap is wide, but the headline blood-pressure category differs under the two major systems described below. The American system places the systolic value in its stage 1 range, while the European system places it in its high-normal range. Recording the complete reading avoids losing that distinction.

A wide pulse pressure can also result from a high systolic value paired with a diastolic value that is not high. That pattern is not the same as two numbers that are both high. The pulse-pressure calculation highlights the gap; it does not replace classification of the two original values.

Narrow pulse pressure: not one meaning

A narrow pulse pressure is often described as less than about 25 to 30 mmHg. This is a practical rule of thumb rather than a universal standard. There is no single narrow-pulse-pressure threshold that determines what a reading means in every person.

The systolic and diastolic values change the context. Compare these two readings:

The first has a low systolic pressure. If it occurs with faintness, unusual weakness, confusion, chest discomfort, or breathlessness, it needs prompt medical attention. The second has a high diastolic pressure and a different clinical question. The same subtraction cannot explain both readings.

A narrow pulse pressure may occur when the heart is ejecting less blood than usual, when circulating fluid is reduced, or when resistance in the arteries is high. It can also appear with an isolated measurement affected by movement, poor cuff fit, an irregular heartbeat, or ordinary variation. These mechanisms are possibilities, not conclusions that can be drawn from the calculated gap.

If a low or narrow result repeats, rest quietly and measure again with the cuff correctly positioned. Keep the full reading and any symptoms together. A persistent pattern is worth raising with your clinician; it is not a reason to change a medicine or dose without medical instruction.

Pulse pressure normal range and age

A value near 40 mmHg is often used as a rough adult reference. It is not a universal normal value, and healthy adults can have readings above or below it. Pulse pressure generally becomes wider over the decades as the large arteries lose some elasticity.

That age-related pattern does not produce a dependable table of normal pulse pressure by age. Major blood-pressure guidelines do not set a simple age-by-age pulse-pressure target for home users. Research studies use different age groups, measurement settings, health definitions, and cutoffs. The evidence supports the direction of the age effect more clearly than it supports precise age bands.

Age-specific context is more defensible for pulse pressure than an age-specific blood-pressure target because arterial stiffness is a plausible mechanism. It still does not justify dismissing a high systolic value as normal for age. Blood-pressure categories and care decisions use the complete reading, repeated measurements, and personal context.

The word “or” matters. If either number reaches a higher row, the reading is generally placed in that higher category. The two frameworks genuinely disagree about where high blood pressure begins: the 2017 ACC/AHA system uses lower thresholds than the ESC/ESH system for several categories. A single reading in any row is not a diagnosis of hypertension. Diagnosis rests on repeated measurements, usually including out-of-office readings, interpreted by a qualified professional. Do not use the table to set or change treatment.

What is more important: blood pressure or pulse pressure?

Blood pressure comes first because systolic and diastolic values are the numbers used by guidelines to classify hypertension. Pulse pressure adds information but does not replace those values.

A pulse pressure of 50 mmHg could come from 120/70, 140/90, or 170/120. The arithmetic is identical, but the systolic and diastolic categories are not. Conversely, two readings with the same systolic value can have different pulse pressures if their diastolic values differ. Keeping the original numbers prevents the calculated gap from hiding that information.

Pulse pressure can draw attention to a pattern of rising systolic pressure and falling diastolic pressure. It can also make a low systolic value or unusually high diastolic value more visible in a diary. Its usefulness increases when it is reviewed alongside the average, spread, time of day, and symptoms.

A single wide reading is not a personal prognosis

A single wide pulse pressure may reflect normal variation, a short-lived change in blood pressure, or a measurement problem. It may also be the first visible part of a real pattern. The number alone cannot identify which explanation applies.

Studies can find an association between wide pulse pressure and cardiovascular risk across a large population. They cannot use one home reading to calculate what will happen to one person. Personal risk also depends on the full blood-pressure pattern, age, medical history, smoking, cholesterol, diabetes, kidney health, symptoms, and other factors. The strength of a population association does not make the individual prediction exact.

The same caution applies to a narrow value. One pulse pressure of 27 mmHg is not a diagnosis. It becomes more informative if it recurs, particularly when it appears with a low systolic pressure, a high diastolic pressure, or symptoms.

How to build a useful pulse pressure diary

Use a validated upper-arm cuff with a cuff size that fits your arm. Wrist devices can be more sensitive to positioning, and a device that has not been independently validated provides less confidence in the result. Validation is a property of the monitor model, not merely of the brand name or the presence of a digital display.

Before measuring, avoid exercise, nicotine, and caffeine for the period specified in your home-monitoring plan. Empty your bladder if needed. Sit with your back supported and both feet flat on the floor. Rest quietly for about five minutes, keep the cuff on bare skin, support the arm at heart level, and remain silent and still while the monitor runs.

Take two readings about a minute apart when your monitoring plan calls for paired readings. Record both rather than keeping only the lower value. Many home protocols use morning and evening measurements for several days, but that is a monitoring structure, not a personal instruction for everyone. Follow the plan provided for your situation.

When a reading needs urgent attention

Very high blood-pressure readings combined with chest pain, breathlessness, weakness, vision change, or confusion mean emergency care, not a diary entry. Seek emergency help rather than waiting for a trend or trying to manage the reading in an app.

Without those symptoms, repeat an unexpectedly high result after resting and check the cuff position and technique. Record both readings. If very high values continue, contact the medical service that manages your blood pressure for advice about what to do next.
